What is a Generic Drug?

A generic drug is a copy of the brand-name drug with the same dosage, safety, strength, quality, consumption method, performance, and intended use. Before generics become available on the market, the generic company must prove it has the same active ingredients as the brand-name drug and works in the same way and in the same amount of time in the body.

The only differences between generics and their brand-name counterparts is that generics are less expensive and may look slightly different (eg. different shape or color), as trademarks laws prevent a generic from looking exactly like the brand-name drug.

    Description

    Altace HCT contains two medications-ramipril and hydrochlorothiazide-for the treatment of high blood pressure. Rampiril is classified as an angiotensin-converting enzyme (ACE) inhibitor and works by dilating the blood vessels, which reduces blood pressure. Hydrochlorothiazide is classified as a thiazide diuretic, or water pill, that causes the kidneys to get rid of excess water and salt through urination.

    Although Altace HCT is used to treat high blood pressure, it should not be the first choice.

    Available only with a valid prescription, Altace HCT comes in oral tablets of the following strengths:

    Routes of administration

    Directions

    Altace HCT should be taken exactly as prescribed by your doctor. If you have any questions about the medication, talk to your doctor or pharmacist.

    Patients are usually recommended to take one tablet by mouth once daily in the morning. The dose may be adjusted in patients with kidney or liver problems.

    Your doctor will tell you how much Altace HCT you should take and may adjust your dose based on how well you tolerate and respond to the medication; however, the dose should not be more than 10 mg ramipril/50 mg hydrochlorothiazide per day.

    Ingredients

    Ramipiril and hydrochlorothiazide are the active ingredients contained in Altace HCT.

    Inactive, or non-medicinal ingredients, include hydroxypropylmethylcellulose, microcrystalline cellulose, pregelatinized maize starch, and sodium stearyl fumarate. The 5 mg/12.5 mg and 10 mg/25 mg tablets also contain red ferric oxide. The 10 mg/12.5 mg tablets also contain red ferric oxide and yellow ferric oxide.

    Contraindications

    Some patients should not take Altace HCT, including those who:

    • Are allergic to another ACE inhibitor, another thiazide diuretic, sulfonamides, or any of the ingredients contained in the medication
    • Have ever had hereditary/idiopathic angioedema, or swelling underneath the skin
    • Are pregnant or breastfeeding
    • Have certain kidney problems, including bilateral renal artery stenosis or unilateral in the single kidney
    • Have low blood pressure or are hemodynamically unstable
    • Cannot produce urine
    • Have severe kidney or liver impairment
    • Are on dialysis
    • Have electrolyte abnormalities


    Patients should not take Altace HCT with sacubitril/valsartan, as the combination can increase the risk of angioedema. You should wait at least 36 hours after taking your last dose of sacubitril/valsartan before you begin taking Altace HCT. If you are switching from Altace HCT to sacubitril/valsartan, wait at least 36 hours after taking your last dose of Altace HCT before you begin taking sacubitril/valsartan. Ask your doctor if you are unsure whether you take any of these medications.

    Patients with diabetes, moderate to severe kidney impairment, high potassium levels in the blood, or congestive heart failure with low blood pressure should not take Altace HCT with medications that contain aliskiren. Ask your doctor if you are unsure whether you take any of these medications.

    Patients who have diabetes with end organ damage, moderate to severe kidney impairment, high potassium levels in the blood, or congestive heart failure with low blood pressure should not take Altace HCT with angiotensin II receptor antagonists (ARBs). Ask your doctor if you are unsure whether you take any of these medications.

    Patients should not undergo extracorporeal treatments, such as dialysis or hemofiltration with certain high-flux membranes and low-density lipoprotein apheresis with dextran-sulfate, if they are taking Altace HCT.

    Cautions

    Altace HCT contains serious warnings and precautions regarding its use during pregnancy. The medication can cause harm or death to an unborn baby; therefore, Altace HCT should be stopped as soon as pregnancy is detected.

    • Before starting blood pressure treatment with Altace HCT medication, tell your doctor if you:
      • Take any prescription or over-the-counter medications
      • Have any allergies
      • Have any health conditions or a medical history
      • Are currently pregnant
      • Are currently breastfeeding
    • Altace HCT can cause a dry, persistent cough. The cough usually goes away after stopping the medication or lowering the dose.
    • Caution should be used when driving, operating machinery, or participating in other activities that require concentration and optimal reaction times.
    • Non-melanoma skin cancer after treatment with hydrochlorothiazide has been reported in some studies. The risk may be greater at higher doses. Patients who take medications that contain hydrochlorothiazide are recommended to get regular checkups on their skin and to immediately report suspicious skin changes. Patients are also recommended to minimize their exposure to the sun, avoid tanning beds, and wear protective clothing and sunscreen with a SPF of 30 or higher when being exposed to the sun or UV light.
    • Altace HCT can cause low blood pressure.
    • ACE inhibitors can cause abnormal white blood cell counts and bone marrow depression. These reactions are more likely to develop in patients with kidney problems or a collagen disease such as lupus or scleroderma, or in patients being treated with other medications that can affect blood cell counts. Patients should seek medical care if they develop signs or symptoms of an infection, such as a fever or sore throat.
    • ACE inhibitors can cause elevated liver enzymes, elevated bilirubin, and liver inflammation. Patients should seek medical care right away if they experience fever, general discomfort, muscle pain, rash, swollen lymph nodes, stomach pain, nausea, vomiting, loss of appetite, itching, or yellowing of the whites of the eyes or skin.
    • ACE inhibitors, including ramipril, have been associated with angioedema, including intestinal angioedema. Patients should seek emergency care right away if they develop swelling of the face, arms, legs, eyes, lips, tongue or have trouble swallowing or breathing. Taking Altace HCT with certain medications, such as mTOR inhibitors (e.g. sirolimus), DPP-IV inhibitors (e.g. sitagliptin), and NEP inhibitors, can increase the risk of angioedema.
    • Patients who take an ACE inhibitor and receive dialysis with high-flux membranes or low density lipoprotein apheresis with dextran sulfate and are taking an ACE inhibitor may experience anaphylactoid reactions, such as nausea, stomach cramps, burning, angioedema, trouble breathing, and severe low blood pressure. 
    • Patients who take an ACE inhibitor and receive desensitization treatment with hymenoptera venom may experience anaphylactoid reactions.
    • Some patients may experience sensitivity reactions to Altace HCT, even if they do not have allergies or asthma. In addition, lupus can be activated or worsened.
    • Patients who take medications that contain ACE inhibitors, including Altace HCT, and receive injectable gold may experience nitroid reactions that can cause symptoms such as flushing of the face, nausea, vomiting, and symptomatic low blood pressure.
    • Abnormal electrolyte levels can occur with Altace HCT treatment.
    • Vision problems, such as glaucoma, can occur with hydrochlorothiazide use.
    • Inform your doctor of any surgeries or procedures you will be having.
    • Photosensitivity can occur with hydrochlorothiazide use.

    Side Effects

    Like with any medication, Altace HCT can cause side effects. The most common include headache, dizziness, and bronchitis. It's important to note that these are not all the side effects that can occur. Call your doctor right away if you have side effects that bother you or won't go away.
