Description
Doptelet (Avatrombopag) is a prescription medication used in patients with thrombocytopenia (low platelet counts). This drug is approved for use in adults with chronic liver disease who need to undergo a scheduled medical or dental procedure. Doptelet is also used in adults and children aged one year or older with persistent or chronic immune thrombocytopenia (ITP) who did not respond well to previous treatments.
Doptelet works by activating thrombopoietin (TPO) receptors in the bone marrow. This prompts the body to make more platelets, which reduces the risk of bleeding and ensures better safety during planned medical procedures.

For Chronic Liver Disease (Procedure Prep)
If you have chronic liver disease and have an upcoming medical procedure, it’s important to start treatment with Doptelet 10–13 days in advance. This allows enough time for your platelet count to rise before your procedure, which should be scheduled 5–8 days after your last dose.
You should take this medication once daily for 5 consecutive days only. If your initial platelet count is below 40 billion per liter (< 40×10?/L), you typically need 60 mg once daily (3 tablets). If your platelet count is between 40 billion and just under 50 billion per liter (40–<50×10?/L), you will likely need 40 mg once daily (2 tablets).
For Chronic Immune Thrombocytopenia (ITP)
Adults and children 6 years of age and older should start by taking one 20 mg tablet once daily. Your doctor will regularly check your platelet count, first weekly and then monthly, to ensure your platelets are at a safe level (=50×10?/L), and that you receive the correct dose. They may adjust your dose if necessary; the maximum is 40 mg (2 tablets) daily.
Children under 6 years old receive only the oral granule form of Doptelet. However, this form is not sold at Canada Pharmacy. Refer to the references provided below for more information.
Doptelet medication should be taken with food for better absorption and effectiveness and swallowed whole. If you forget a dose, take it as soon as possible, but never take 2 tablets at once. Continue with your next scheduled dose as usual if it’s almost time for it.

Cautions
Always inform your doctor of your complete medical history, including health conditions, medications, allergies, lactose intolerance, pregnancy, and breastfeeding status, before taking this medication.
Taking Doptelet may put you at an elevated risk for blood clots. Schedule regular blood work, watch for symptoms, such as sudden swelling, leg pain, chest pain, trouble breathing, a sudden severe headache, or vision changes, and seek medical help immediately if you suspect a blood clot.
Chronic liver disease or chronic immune thrombocytopenia raises your blood clot risk. The same applies to medical conditions, such as Factor V Leiden, Prothrombin gene mutation, Antithrombin deficiency, Protein C or S deficiency, or antiphospholipid syndrome. Use caution and inform your doctor if you have any of the above.
Keep in mind that Doptelet is not meant to normalize your platelet levels but ensure that they are in a safe range. Raising platelet levels too high increases your risk of clotting.
Doptelet is not recommended during pregnancy or when breastfeeding as it may cause fetal harm or pass into breast milk. Wait 2 weeks after your last dose before breastfeeding.
Doptelet is not approved for the treatment of thrombocytopenia in children with chronic liver disease.
Certain medications that affect CYP2C9 or CYP3A4 (such as fluconazole or rifampin) may change how Doptelet works. Inform your doctor if you take any of them, so your dose may be adjusted if you are being treated for ITP.
Closer monitoring may be needed in patients with liver or kidney impairment, as drug levels may be elevated.
After you discontinue Doptelet, your platelet counts may drop below baseline. Since this can cause a potentially dangerous bleeding risk, your doctor will monitor you closely.
Contraindications
According to the FDA (U.S.), there are no known contraindications of Doptelet (Canadian labelling lists hypersensitivity as a contraindication).
Side Effects
Common and manageable Doptelet side effects may include headache, fatigue, nausea, abdominal pain, fever, bruising, gum bleeding, nosebleeds, and upper respiratory infections.
More serious but less common side effects may include blood clots, dangerously low sodium levels, severe allergic reactions, very high platelet counts, viral infections, severe or persistent headache, and rare cases of high white blood cell count (leukocytosis).
