Description
Itovebi (Inavolisib) is a targeted cancer therapy used exclusively as part of a three-drug treatment plan, combined with palbociclib and fulvestrant. Itovebi treats hormone receptor–positive (HR+), HER2-negative advanced, or metastatic breast cancer with a PIK3CA gene mutation. It’s used in adults whose cancer has become resistant to hormone therapy. Itovebi blocks the PI3K-alpha enzyme, which cancer cells with the PIK3CA mutation rely on to grow and survive. This ultimately helps slow tumor growth.
Directions
You can buy Itovebi in 3 mg and 9 mg strengths. The Itovebi drug is available as an oral tablet and should be taken exactly as prescribed. It is always administered in combination with palbociclib and fulvestrant, not as a monotherapy. Your doctor may also prescribe additional medications as part of your treatment plan to lower your hormone levels and improve the effectiveness of Itovebi.
9 mg once daily is the typical dose of Itovebi. However, 6 mg or 3 mg is administered when a lower dose is needed due to a patient’s decreased kidney function, intolerable side effects, or other factors. Itovebi is swallowed whole with or without food, but chewing, crushing, or splitting the medication must be avoided.
This medication should be taken at approximately the same time each day. If you miss a dose, take it as soon as possible within nine hours. If nine hours have passed, skip it and take the next scheduled dose. If you vomit, do not take an additional dose and resume with your regular dosing schedule the next day.
Itovebi is taken as long as it is effective and side effects are tolerable and safe.
Ingredients
The active ingredient in Itovebi is inavolisib.
Inactive ingredients are lactose, magnesium stearate, microcrystalline cellulose, sodium starch glycolate, iron oxide red, iron oxide yellow (9 mg tablet only), macrogol/polyethylene glycol, polyvinyl alcohol (partially hydrolyzed), talc, and titanium dioxide.
Contraindications
According to the FDA, Itovebi has no contraindications. However, every patient is assessed individually.
Cautions
- Before taking Itovebi, inform your doctor of
- your complete medical history, especially diabetes, kidney or liver issues, heart conditions, digestive and mouth disorders, prior cancer treatments, allergies, pregnancy, breastfeeding status, and fertility concerns.
- all the medications you’re taking, including over-the-counter drugs and herbal or vitamin supplements.
- Avoid pregnancy during treatment as Itovebi can harm an unborn baby. Use non-hormonal birth control and immediately inform your doctor if you become pregnant.
- Do not breastfeed while taking Itovebi or for at least 1 week after your last dose.
- Do not take Itovebi if you’re allergic to one of its ingredients. Seek immediate medical care if you experience allergy symptoms such as rash, itching, swelling, or trouble breathing.
- Inform your doctor if you notice unusual thirst, frequent urination, or blurred vision, as Itovebi may increase blood sugar levels.
- Contact your doctor if your mouth becomes painful or swollen, since Itovebi can cause soreness or ulcers. You may be prescribed a mild steroid mouthwash to prevent or reduce this side effect.
- Consult your doctor if you experience persistent diarrhea and stay hydrated, since this side effect may lead to dehydration or kidney problems when severe.
- Your doctor will monitor your blood sugar, liver function, and electrolyte levels using blood tests, and may adjust your Itovebi dose or that of other medications if needed.
- You may need a lower dose of Itovebi if you have severely reduced kidney or liver function, since the medication might leave the body more slowly, affecting your Itovebi levels in the blood.
Side Effects
Common, but usually manageable side effects of Itovebi when taken as part of its three-drug treatment plan include high blood sugar, fatigue, mouth sores, diarrhea, nausea, vomiting, appetite loss, skin rash, and dryness. Less common but severe side effects requiring medical attention include severe hyperglycemia or infections, increased liver enzymes, allergic reactions, or fetal harm.
