Ogivri (Trastuzumab)

Description

Ogivri is a prescription cancer treatment used in adults. It treats certain types of cancers that have high levels of the HER2 protein (Human Epidermal Growth Factor Receptor 2). For example, it may reduce the risk of early breast cancer returning after surgery. Additionally, Ogivri is effective in treating metastatic breast cancer as well as metastatic stomach and gastroesophageal junction cancer.

Since the HER2 protein drives cancer development, targeting it directly can slow or stop tumour growth. The medication works by binding to the HER2 protein on cancer cells and blocking signals that drive cancer growth. Additionally, Ogivri helps your immune system recognize, weaken, and destroy cancer cells.

Directions

You can buy Ogivri in the following strengths:

• 420 mg multiple-dose vial with a vial of sterile diluent included
• 420 mg multiple-dose vial (powder only; no diluent included)

This medication is only given by IV infusion in a medical setting. Never administer Ogivri on your own and always follow your doctor’s advice. A healthcare professional will first mix it and then give it to you slowly through a drip.

Ogivri infusions usually take 30 to 90 minutes, depending on whether it is your first dose or a follow-up dose. Your dosing schedule and treatment length vary depending on the type of cancer being treated.

Early (Adjuvant) Breast Cancer

There are two standard dosing schedules for treating early (adjuvant) breast cancer. If your healthcare provider prescribes weekly dosing, you will usually receive a 4 mg/kg loading dose initially, followed by half the weekly dose (2 mg/kg) during specific chemotherapy regimens.

If your doctor opts for treatment every 3 weeks, your first dose (loading dose) will typically be 8 mg/kg. Afterwards, when Ogivri is used alone after chemotherapy, 6 mg/kg is administered every 3 weeks. The treatment generally continues for up to a year.

Metastatic Breast Cancer

Metastatic breast cancer is usually treated weekly, with the initial loading dose being 4 mg/kg and 2 mg/kg following weekly. This dosing schedule is typically followed until the cancer progresses.

Metastatic Gastric / GEJ Cancer

Patients receive treatment every 3 weeks. However, the initial (loading) dose will be 8 mg/kg. Afterward, patients will receive 6 mg/kg every 3 weeks until the disease worsens.

No matter the form of cancer, if you missed a dose by 1 week or less, your healthcare provider should administer it to you as soon as possible. If you missed it by more than one week, the full loading dose should be repeated.

Cautions

Always inform your doctor of your complete medical history, including health conditions, medications, allergies, pregnancy, and breastfeeding status.

Only take Ogivri exactly as prescribed and never substitute it with another HER2 medication.

Ogivri is only effective when treating HER2-positive cancers, which must be determined with an FDA-approved test. Confirm this status before beginning treatment.

Regular medical monitoring is essential when taking Ogivri. This may include blood tests and physical exams.

Inform every healthcare provider that you are receiving Ogivri. This is especially crucial before surgeries or taking new medications.

Ogivri may negatively impact your heart, making regular heart function tests essential during treatment. Tell your doctor about prior anthracycline (e.g., doxorubicin) treatment, since the risk of heart problems is higher when these drugs are used before or with Ogivri.

Your doctor will watch you closely during infusions, especially your first one. This is to avoid severe infusion reactions, including trouble breathing and allergic reactions.

Avoid pregnancy as Ogivri can harm an unborn baby. If you are a female of reproductive age, you must have a pregnancy test before starting and use effective birth control during treatment and for 7 months after the last dose. Do not breastfeed for 7 months after your last dose.

Contraindications

According to the FDA, there are no known contraindications of Ogivri.

Side Effects

Patients most frequently report fever, chills (especially during early infusions), nausea, vomiting, fatigue, or weakness. Diarrhea, rash, mild skin reactions, a cough, runny nose, body aches, or muscle pain are also common.

More serious and typically rarer side effects may include heart problems like weakened heart function or heart failure. Signs may include shortness of breath, swelling, and rapid weight gain. Trouble breathing, wheezing, or low blood pressure may also indicate severe infusion reactions, while swelling, hives, and difficulty breathing may accompany severe allergic reactions. Other severe side effects requiring immediate medical attention may include serious lung problems, such as inflammation or breathing difficulty or severe diarrhea or dehydration.
