What is a Generic Drug?

A generic drug is a copy of the brand-name drug with the same dosage, safety, strength, quality, consumption method, performance, and intended use. Before generics become available on the market, the generic company must prove it has the same active ingredients as the brand-name drug and works in the same way and in the same amount of time in the body.

The only differences between generics and their brand-name counterparts is that generics are less expensive and may look slightly different (eg. different shape or color), as trademarks laws prevent a generic from looking exactly like the brand-name drug.

Generics are less expensive because generic manufacturers don't have to invest large sums of money to develop a drug. When the brand-name patent expires, generic companies can manufacture a copy of the brand-name and sell it at a substantial discount.

    Description

    VIZZ (aceclidine ophthalmic solution) 1.44% is the first and only FDA-approved aceclidine-based eye drop designed to treat age-related blurry near vision (presbyopia) in adults. Administered once daily, it works within 30 minutes and delivers near-vision improvement that lasts up to 10 hours.

    Aceclidine acts as a predominantly pupil-selective miotic, constricting the iris sphincter muscle to create a “pinhole effect” that extends depth of focus. This improves near vision without causing a myopic shift and with minimal stimulation of the ciliary muscle.

    Clinical trials (the CLARITY studies) demonstrated robust efficacy, with significant near-vision improvements as early as 30 minutes post-dose and sustained results for up to 10 hours, alongside a favorable safety profile.

    Routes of administration

    Ophthalmic (eye drops)

    Directions

    • Instill one drop in each eye once daily. Wait two minutes, then instill a second drop in each eye from the same single-dose vial.
    • Contact Lenses: Remove lenses before applying; they may be reinserted 10 minutes after use.
    • Multiple Ophthalmic Products: If using other eye drops, administer them at least 5 minutes apart.
    • Handling: Do not touch the vial tip to the eye or any surface to avoid contamination or injury. Discard the single-dose vial immediately after use.

    Ingredients

    • Active: Aceclidine hydrochloride 1.75% (equivalent to aceclidine 1.44%) – approximately 17.82 mg per mL.
    • Inactive: Polysorbate 80, mannitol, hypromellose, edetate disodium dihydrate, sodium citrate dihydrate, water for injection. May contain hydrochloric acid or sodium hydroxide to adjust pH to 4.5–5.5.
    • Formulation: Sterile, clear to opalescent, colorless to slightly yellow, preservative-free ophthalmic solution in single-dose vials.

    Interactions

    • Allow a 5-minute interval between VIZZ and any other topical ophthalmic medications.
    • Remove contact lenses before applying. Lenses may be reinserted 10 minutes after instillation.

    Cautions

    • Blurred or dim vision may occur temporarily. Avoid driving or operating heavy machinery until vision clears.
    • Retinal tear or detachment has been reported with other miotic agents in predisposed patients. A retinal examination is recommended before starting treatment. Seek immediate care if you notice flashes of light, floaters, or sudden vision loss.
    • Iritis: May worsen inflammation in patients with a history of iritis or synechiae.
    • Hypersensitivity: Do not use if allergic to aceclidine or any component of the formulation.
    • Contamination risk: Avoid contact of the vial tip with the eye or any surface. Discard the vial immediately after use.

    Side Effects

    In clinical trials, the most common adverse reactions (occurring in 5% or more of patients) included:

    • Instillation site irritation (about 20%)
    • Dim vision (about 16%)
    • Headache (about 13%)
    • Conjunctival hyperemia (about 8%)
    • Ocular hyperemia (about 7%)


    Most side effects were mild, temporary, and resolved without treatment.
