Description
Xhance (Fluticasone propionate) is a prescription corticosteroid nasal spray used in adults. It treats chronic rhinosinusitis, with or without nasal polyps. In doing so, this medication helps reduce inflammation, swelling, mucus buildup, and irritation in the nasal and sinus tissues. It can also reduce polyp size. Over time, Xhance reduces associated symptoms and prevents their recurrence. More specifically, it improves breathing and decreases nasal congestion.
Directions
You can buy Xhance nasal spray as a 16 ml bottle, which provides 120 metered sprays after priming. Xhance should be taken exactly as prescribed. One spray delivers 93 mcg of fluticasone propionate (per nostril), which adds up to the recommended dose of 186 mcg per dose. Since this medication is typically prescribed for twice daily use, most patients will use an Xhance dosage of 372 mcg daily. In some cases, your doctor will prescribe 2 sprays per nostril twice daily, for a total dose of 744 mcg. For optimal effects, use Xhance regularly. The onset time and degree of symptom relief may vary from person to person.
Ensure you shake Xhance before each use and prime it before the first administration. Press the bottle 7 times and spray into the air, away from your face. Similarly, repeat this process, spraying only twice if Xhance has not been used for over 7 days.
To ensure the active ingredient is delivered into the nasal passages, administer one pump into one nostril while simultaneously exhaling into the device’s mouthpiece. Follow the steps below:
- Insert the tip of the nosepiece into one nostril and form a tight seal between the nosepiece and the nostril.
- Place the flexible mouthpiece into the mouth. Bend it as necessary to maintain a tight seal with the nostril.
- Blow into the mouthpiece. Simultaneously, push the bottle up to activate the spray pump.
- Continuing to blow through the mouth. Do not inhale or exhale through the nose when you spray.
- After spraying, you may breathe normally.
- Avoid spraying directly on the nasal septum.
- Repeat the process in the other nostril for a full dose.
Ingredients
The active ingredient in Xhance nasal spray is fluticasone propionate.
Inactive ingredients are microcrystalline cellulose, carboxymethylcellulose sodium, dextrose, benzalkonium chloride, polysorbate 80, edetate disodium dihydrate, sodium hydroxide and hydrochloric acid (to adjust pH), and purified water.
Contraindications
People with hypersensitivity to any ingredient in Xhance must not take this drug.
Cautions
- Before taking Xhance, inform your doctor of:
- Your complete medical history, especially nasal and vision problems, current or past infections, tuberculosis, immune system problems, adrenal gland problems, and osteoporosis.
- All the medications you’re taking, including over-the-counter drugs and herbal or vitamin supplements. This is to avoid serious drug interactions since some medications can significantly increase steroid levels in your body when used with Xhance. This may include certain HIV drugs, antifungals, or antibiotics.
- Your allergies. Do not take Xhance if you’re allergic to one of its ingredients. Seek immediate medical care if you experience allergy symptoms such as rash, itching, swelling, or trouble breathing.
- When taking Xhance nasal spray, watch for signs of nasal septal perforation (a hole in the nasal cartilage). Hearing a whistling sound when breathing or new nasal pain may indicate this rare but serious condition. Schedule regular check-ups with your doctor during long-term use.
- Watch for candida (yeast) infections in your nose, throat, or mouth, and report redness, soreness, or white patches to your doctor.
- Do not use Xhance if you recently had nasal surgery, sustained a nasal injury or have an open sore, as this medication can slow the healing process. Don’t start this medication until your doctor confirms healing.
- Your risk of developing glaucoma and cataracts may rise when using Xhance. Tell your provider immediately if you notice blurred vision, eye pain, or other vision changes and attend regular eye exams.
- Xhance may impact your immune system and raise your infection risk. Try to limit exposure, and use Xhance with caution or avoid Xhance if you have certain infections, including tuberculosis, fungal infections, or ocular herpes. Contact your doctor if you notice signs of infection, such as fever or fatigue.
- Use Xhance with caution if you are sensitive to steroids and never use more than the recommended dose. Both may affect your adrenal glands. Signs may be extreme tiredness, weakness, nausea, vomiting, or low blood pressure. Stress and stopping oral steroids may also contribute to reduced adrenal function.
- Long-term steroid use may decrease bone mineral density. Monitoring, or preventive treatment, may be needed in patients with increased risk for bone loss (e.g. postmenopausal women, smokers, people with poor nutrition, or those taking certain other medicines).
- Corticosteroids may slow growth. Xhance is not approved for use in children and has not been studied in this age group.
Side Effects
Common but usually manageable side effects of Xhance include nosebleeds, nasal congestion, redness and irritation of the nasal passages, mild sores or ulcers inside the nose, headaches, sore throat or nose, and swelling in the nose or throat. More serious side effects include eye problems (e.g. glaucoma, cataracts), severe allergic reactions, infections, nasal septal perforation, reduced adrenal function (extreme tiredness, weakness, nausea, low blood pressure), and poor nasal wound healing.
