What is a Generic Drug?

A generic drug is a copy of the brand-name drug with the same dosage, safety, strength, quality, consumption method, performance, and intended use. Before generics become available on the market, the generic company must prove it has the same active ingredients as the brand-name drug and works in the same way and in the same amount of time in the body.

The only differences between generics and their brand-name counterparts is that generics are less expensive and may look slightly different (eg. different shape or color), as trademarks laws prevent a generic from looking exactly like the brand-name drug.

Generics are less expensive because generic manufacturers don't have to invest large sums of money to develop a drug. When the brand-name patent expires, generic companies can manufacture a copy of the brand-name and sell it at a substantial discount.

Dapagliflozin is classified as an SGLT2 inhibitor, helping eliminate glucose from the bloodstream through the kidneys. Metformin, on the other hand, is a biguanide that reduces glucose production in the liver and improves the body's response to insulin. This medication may be prescribed to improve blood sugar management in adults with type 2 diabetes mellitus alongside diet and exercise.

    Directions

    Patients should take Xigduo XR orally, following the prescribed dosage by their healthcare provider. This medication is typically available in tablet form once daily in the morning with food. It's important to swallow the tablets whole and avoid crushing or chewing them.

    For individuals not already using Dapagliflozin, the usual starting dose is 5 mg once daily, which can be adjusted as needed. If transitioning from Metformin extended-release, patients should skip their last dose of Metformin before starting Xigduo XR. Alongside this medication, maintaining a balanced diet and regular exercise is recommended for effective diabetes management.

    Contraindications

    Avoid taking or using this medication if any of the following apply:

    • Individuals with severe kidney impairment or on dialysis
    • Those with a known hypersensitivity to Dapagliflozin or Metformin, such as a severe reaction or rash
    • Individuals with a condition called metabolic acidosis, including diabetic ketoacidosis

    Cautions

    • Before starting treatment with Xigduo XR, be sure to inform your healthcare provider about the following:
      • Any medical conditions you currently have
      • Any allergies
      • All medications you are currently taking
      • Whether you are pregnant or breastfeeding
    • Monitor for signs of lactic acidosis, a serious condition that can occur with this medication.
    • If you have type 1 diabetes or are at risk for ketoacidosis, monitor ketone levels. Stop Xigduo XR if ketoacidosis is suspected and resume only after the condition is resolved.
    • Before starting Xigduo XR, ensure proper hydration, especially in older adults, those with kidney issues or low blood pressure, or if using diuretics.
    • Watch for symptoms of urinary tract infections, including urosepsis or pyelonephritis, and treat promptly if these infections occur.
    • Decreasing the doses of insulin or insulin secretagogues might be necessary to prevent low blood sugar when used with Xigduo XR.
    • Be aware of the risk of necrotizing fasciitis of the perineum (Fournier’s Gangrene); seek immediate medical care if you experience symptoms such as genital or perineal pain, swelling, redness, fever, or malaise.
    • Metformin in Xigduo XR may cause vitamin B12 deficiency. Check blood levels annually.
    • Monitor for genital mycotic infections and treat if necessary.

    Side Effects

    Xigduo XR side effects may include yeast infections, both vaginal and penile, presenting symptoms such as itching, discharge, and discomfort. Other frequent side effects include urinary tract infections, headaches, nasal congestion, sore throats, diarrhea, and general feelings of fatigue.

    Xigduo XR can cause significant health issues including dehydration, which may lead to dizziness or fainting, especially when standing up quickly. The risk is higher among older adults, those with kidney issues, or when using certain medications like diuretics. Serious urinary tract infections and low blood sugar episodes are also risks, particularly when Xigduo XR is taken with other diabetes medications. In rare cases, it can cause a life-threatening bacterial infection known as necrotizing fasciitis of the perineum, which may require urgent medical attention. Long-term use may lead to vitamin B12 deficiency. Prompt medical intervention is important if serious symptoms like severe allergic reactions or signs of necrotizing fasciitis occur.
